Training Record Books

Years 1 & 2

The Training Record Book should be used to record competencies, information and instructions are available in the book, please click on the link - CT12TrainingRecord2009

Regular workplace based  assessments are used to describe progress throughout the training programme - see below for blank forms.
 
Assessments to be used during intensive care medicine training are found on the Royal College website - http://www.rcoa.ac.uk/index.asp?PageID=1066
  

Years 3 & 4

In years 3 and 4 trainees use the blue Training Record Book ST3-4 - click on the link - TrainingRecord3-4 (large document to download) or TrainingRecord34(zipversion) completing the section for each module as well as the generic skills. Additional DOPs, Anaes-CEXs and CBDs are available as above.   If you require a paper copy of this book please  email Julie Flowers.

Trainees should plan to complete the whole book but the sections on imaging, maxillofacial/dental, ophthalmic surgery, plastics/burns and miscellaneous are 'additional units' if you are looking on the college website at the syllabus for ST3-4.  If you feel that you are having any difficulty getting the necessary experience to fill out the book please speak to your College Tutor. 
 

Year 5

Year 5 trainees use the green training Record Book (ST 5,6,7 and SpR 3,4,5) for any modules completed as well as for generic skills.  Please click on the link - ST567TrainingRecord2009.  Anaes-CEXs and CBDs forms below.

SpRs

SpR 3, 4, 5 trainees should continue to record their competencies in the green booklet, click on 'ST5,6,7 Training Record 2009' (above).  You would be wise to include some new style WBAs. Forms below.
